Abstract
Tertiary-level psychiatric care in the Caribbean under the British Colonial Empire was intended to provide protection from environmental conditions and serve as a custodial mode of treatment. The psychiatric field advocated moral treatment and work as therapy; however, most first-world countries have eliminated vocational care as part of the care for chronic mental health services. In a visit to Cuba in 1975, the authos observed that the Cubans had put all of their mental hospital patients to work. Patients tended a 100-acre rose plantation. This model led to a critical apprraisal of the setting of institutionalization at the Bellvue Hospital in Jamaica and the author designed a schema, which involved a graded rehabilitation programme.....
